NDIS Assistance Program
The Junction Works NDIS Assistance Program can support you with applying for the NDIS, assisting you to understand and utilise your NDIS Plan and support you with NDIS reviews.
Our experienced Support Coordination team can provide you with a free one-off service that includes:
- NDIS information and advice
- Assistance with NDIS review or access applications
- Quotes
- Risk and Support Assessments
- Connection to providers and services
Our team are also available for information and group sessions including school visits and disability events.
We can support you by:
- Guiding you through NDIS Processes
- Understanding and utilising your NDIS Plan and funding
- Providing information to help you negotiate services and service agreements
- Connecting you to providers and services that meet your individual needs
- Supporting you through transitions and life-stage planning

What is Support Coordination?
Our friendly and experienced team of Support Coordinators help you understand your NDIS Plan, connect with the right services, and make sure your supports work well together.
We can guide you through using your funding, help resolve any issues with providers, and build your confidence to manage your supports more independently over time. We also assist with preparing for NDIS Plan reviews by gathering reports and explaining what additional supports you may need.
What funding do I need?
“Capacity Building – Support Coordination.” In some cases, Support Coordination may also be delivered under Specialist Support Coordination funding if you have more complex needs.
What are the eligibility criteria?
NDIS participants aged 18+ with the relevant funding are eligible to access Support Coordination services. Support Coordination is designed to assist people:
- with complex support needs or changing needs
- with limited informal supports
- going through a significant life transition
- at risk of homelessness or substance misuse
- who are connected with the justice system
- from a CALD (Culturally and Linguistically Diverse) background
- who manage multiple NDIS Plans or are new NDIS participants
Do you provide in-kind Support Coordination services?
Yes. If your NDIS plan doesn’t include Capacity Building funding, we can offer short‑term, no‑cost Support Coordination to help you get started, advocate for your needs, and work toward securing the appropriate funding in your next Plan review.
Please note our availability of in-kind services is dependent on our current capacity.
Where do you provide services?
Throughout South-West Sydney, with offices located in Austral and Busby.
Do you provide support online or over the phone?
Yes! We provide in‑person Support Coordination services within our local government area and can travel up to 30 minutes beyond this. If in‑person support isn’t suitable, we also offer phone and online (telehealth) appointments for your convenience.
Do you provide bilingual services?
Yes, we can provide bilingual Support Coordination services in Vietnamese.

Can Support Coordinators help me to get more funding?
If you require additional funding for a new service or support, we can request a Plan Review with the NDIS. We work closely with you and your care team to gather the necessary reports and assessments to demonstrate why the extra supports are reasonable and necessary.
Can Support Coordinators help to me understand the upcoming NDIS changes?
Yes. Whilst the NDIS is still releasing information and finalising details, our qualified and experienced team of Support Coordinators stay up to date with official announcements and can explain what is known so far. We can help you make sense of reforms, understand which changes are confirmed, and identify what is still being developed.
As new rules, funding structures, and registration requirements are introduced over the coming years, we can guide you through each step, help you prepare for Plan reviews, and ensure you feel informed and confident. Our role and focus are to support you, provide clarity, and help you navigate the NDIS with confidence!
