Disability Case Management and Brokerage Program

Linking people with a disability with what they need.

Often it is difficult to find the way through the maze of services and information to find what is available for families and people who have a disability.

The Disability Case Management and Brokerage program aims to meet the needs of people with a disability aged 12 and older who need the additional support of a case manager. This can be for many reasons, such as mental health issues, homelessness, drug or alcohol dependence/abuse and violence.  This program works intensively in complicated and multilayered situations to facilitate each person achieving the goals that are most important to them.

Each person has an allocated case manager to work with them, identifying needs and establishing a client centred plan with goals. They provide direct support, advocacy and links to other services ensuring the case plan goals are achieved.
