Pathways to a brighter future

The Community Participation Program provides a person centred planning approach to social inclusion for people with a disability who have moderate to high support needs and who need an alternative to employment.

Person-centred planning focuses on the person and their needs by putting them in charge of defining the direction for their lives. The aim is to ultimately lead to greater inclusion as valued members of both community and society.
